The numbers are in...

The results of H's SA came today.

Some of the data I understand but the others I don't....particularly the morphology numbers.
Count is 39 million
Motility 52%
Progression 2.5
Morphology -
Normal oval 5%
Amorphous 47%
Small head 4%
Tapered 31%
Double tail 2%
Coiled tail 10%
Blunted tail 1%
WHO morphology > 30% (normal oval)

There is a second 'summary' page with a bunch of boxes, there are 2 checked off - one saying 'abnormal' and the other says 'morphology (teratozoospermia) (<4% by strict criteria; <12% who criteria)

Before I completely freak out, does anyone understand what this even means? The nurse who gave the results over the phone said this (morphology) was not a problem but it seems like it is if it's been almost 10 months of no success.

Should I go ahead and start testing for myself? I mean, the OPKs I've been using show a clear surge so I'm pretty certain I'm ovulating. I've been temping too but I'm still new to it so the last few months have been hard to pinpoint actual date of O (due to traveling, crazy sleep schedule)

Feeling slightly sad right now :(

Re: The numbers are in...

  • I *think* that second page is telling you that abnormal morph would be below 4% by the strict criteria and below 12% by the WHO criteria.

Based on that assumption, your H's morph of > 30% is totally fine.

But that's just my interpretation; you should definitely have a follow-up for the dr to clearly explain it to you.

  • Of course he's going to follow up with the doctor....we came home from work last night and these came in the mail.
The >30% I'm not quite sure if those were part of the results because they weren't checked off in a box or anything....just underneath the boxes with the other numbers. I'm most concerned about the boxes that were checked on the summary page.

And as far as testing for me, and to the person who said 10 months isn't that long, I realize that for some, this isn't a long time, however, I am turning 40 next year, and as you know, the older you get, the more potential issues you have. So yes, I might need testing for myself too. I've already accepted this fact. I probably didn't mention my age at any point since I've posted in here, but yes, I am not in my 20s, I'm almost 4-0.

It may just be timing, but my H and I have been banging it out a lot (no further details needed, just know it's been more than enough) so 10 months of nothing, with my age and now potentially this, I'm thinking there might be something wrong.

Thanks for being so sensitive in your post, btw....really didn't need that. But guess I should expect that from this board.

No one was insensitive. It was all good advice based on many years of experience. The nurse told you the results were fine. Aim was just encouraging you not to borrow trouble. And I just re-read the prior posts - no one said 10 months wasn't long. It seems you have just worked yourself into a tizzy and want to blame other people.

Since you are 40, you should have testing. It is no one on this board's fault that you never mentioned that. FWIW, I was 30 and ovulated like clockwork temping and using O predictors each month for over a year. However, testing showed that I had severe DOR with undetectable eggs and an ovarian age of about 48. Although my DH also had low morph, my condition was what gave us only a 10% chance of conceiving with an IUI and injects.

Thus, even though DH's testing would have given us a "reason" for out difficulty, if I did not also get testing, we would not have had the correct diagnosis or treatment.

That being said, if you don't want helpful advice, don't ask for it on a public forum.

    Good luck.

Yep. Can't know things if you don't mention them. Being 4-0 rather than in your 20s or 3-0s does change things.

And also, thanks for understanding that having all details about your history is relevant. I stand by everything I said about 10 months and timing. It is usually not a case of trouble, but a case of impatience. With your age, it may be trouble but it may not be. Best of luck at the follow ups with the doctor.

  • Yea, sorry, I thought I did mention my age when I initially started posting in this area of the forum. I'm not blaming anyone, obviously and sorry if it seemed that way, but please understand why I might be a bit alarmed when I see some papers with the word 'abnormal' checked off and no real explanation. I just posted here because I was hoping that someone else who has had similar results or experience could shed some light on what I'm reading before we do the follow up so I might have a better idea of what questions we should be asking.

I'm not really impatient either, even though it might seem that way, but I'm trying to be mindful of the fact that I am slightly on the older side of this TTC journey so I don't want to waste too much time trying to figure out what, if anything, is wrong. I know we'll figure things out, but it's just the longer it's taking, the more disheartening it is. Overall I'm fine, but I occasionally have broken down crying...and it sucks feeling this way :/

Thanks for understanding and sorry if I came across as rude.
  • Age matters. If you are 35+ then dr's usually move to do testing after 6 months of ttc, rather than one year. They will want to see what the quality is of your eggs and such.

    Nobody was insensitive.

  • Yes definitely get yourself tested. TTC involves 2 people and regardless of your DH's diagnosis (although overall his numbers don't seem that bad to me) you need all the possible information about yourself to correctly make a treatment plan.
I believe testing is recommended after 6 months of TTC for ladies over 35.

Like Aim said don't borrow trouble until you know but it's definitely worth finding out all the information. As I said below my DH's morph was 2% and that wasn't the issue for us. I wasn't ovulating regularly so regardless of that we weren't going to get pg until I ovulated.

CCH.. I'm butting in because I fall in to the 35+ group, but this honestly depends on your dr. It took me 10 months to conceive DS. At 7 months my dr ordered labs and said we'd wait until we hit the year mark, even though I was 36. So not all doctors go by the old stigma of 35 being a near death sentence for infertility unless you have underlying background that may say otherwise. Of course there are the other doctor's that are quick to lump AMA for the extra testing and ultimately $$. But overall if one is healthy, not all do. Butting out..

Speak sweetly so that if you ever have to eat your words, they don't taste bad.

    Lesson learned!!

  • Certainly can't hurt her to ask her dr or reconsider feeling like she needs to wait until it has been a year. Says the girl that was under 30 and absolutely took longer than 10 months to conceive and only did so thanks to treatments.

  • Resolve and the National Infertility Association both say that after 35, 6 months is the red flag. It's 12 months for under 35.

OP, I'd do the testing. It could be nothing but is definitely worth peace of mind in my opinion.

    Best wishes to you.

  • Thanks everyone. I called H's doctor yesterday but they said he's out until Monday and will call me back. Once I speak to him though, I'm going to start looking into testing for myself. Hoping it's really just nothing....
