I found it! The GP Am-I-Pregnant Quick reference guide for newbs
Bruins post about the newbs made me remember this handy dandy reference guide I created many moons ago. Turns out I had it saved to my old bio.. haha.. All you young whippersnappers who weren't here years ago won't get some of the stupidity on it (howler monkeys and "prago") but whatever. 
I really should update it so it's more applicable to this GP generation

T-TTC since Dec 2008. PCOS/nonexistant cycles(anovulation) and endo. HSG in '10 revealed both tubes blocked. Lap surgery in Dec '10 to correct. Failed Clomid/IUI and injectable(Bravelle)/IUI cycles so far.
