It's getting real.

I went to my OBGYN for my yearly today. She told me in September that if I wasn't pregnant by this appointment that we would start testing and clomid.

She was true to her word. She originally said that we would just start clomid, but I rejected, and told her not without an HSG first. (Thank you ladies!)

So now I schedule a pelvic ultrasound and HSG soon. I am planning on doing them the same day so I don't have to make two trips. I am also trying to find somewhere for DH to do his SA.
Question- what is the pelvic US for? Is that just to see how things look?

I asked her about monitoring when I start clomid and she just said one US after I finish the dose and if that one shows no problems then that is it. Because she is planning on starting me out on a low dose. Does this sound normal?

Another question. What is the difference between clomid and femera, is there one that might be better/ have less side effects?

I feel so funky after this appointment. Relief that we are finally getting somewhere with testing and an action plan. Disappointment that we are even at this point. Anger at the MA that found it necessary to tell me how she just got pregnant after she stopped trying.

  • The u/s is probably checking your lining and looking for any obvious issues. Standard protocol there. Good for you for insisting on an HSG!

    So just one monitoring appt? Will they do another one prior to O or IUI (if you're doing that). I'd ask for more, but that's just me.... They started me low, but I didn't respond at first so if I hadn't had multiple ultrasounds, we wouldn't have known.

    My RE doesn't do Femara since it's not technically an "infertility drug" and is actually for breast cancer (kind of a CYA deal for them) so I have no experience with it or the side effectss, but there are definitely side effects with Clomid. They don't call it the Clomid Crazies for nothin'! ;) I've heard good things about Femara though and I would've tried it if my RE used it. Good luck with all of this!

    T-TTC since Dec 2008. PCOS/nonexistant cycles(anovulation) and endo. HSG in '10 revealed both tubes blocked. Lap surgery in Dec '10 to correct. Failed Clomid/IUI and injectable(Bravelle)/IUI cycles so far.

  • The us would be to check that there are no cysts and to get a baseline of where you are at on CD3. You should have one at the start of each medicated cycle.

    As far as monitoring, all of my Clomid cycles involved inject and trigger. So I went CD3 for us/bw, Clomid CD3-7, Bravelle CD9, and then monitoring CD11. If there was appropriate growth, I'd get my Trigger instructions. If not, I'd come back for follow up us/bw until I was ready.

  • I think the U/S is to look at the lining, look for growths, cysts, blockages, etc.
    I'm glad you have a plan and that the appointment made you feel better!

  • I always got an u/s as a baseline and then another one a certain number of days after finishing the dose.  I would ask for more u/s though if you can.  I went for one and it showed no response really, so I had to come back for more.  I didn't respond at all to Clomid, but I did to Femara.  I never really experienced any side effects with either of them though.

    I forgot to ask about CD3! I knew there was something else I wanted to ask her about. I'll just call for them to fax the req on CD1.

    She said I could do the US on any day.

    @RockABye, I have already warned DH of the clomid hormones!
    I will not be doing IUI.
    I will ask her about more monitoring, you made very good points.

    Yes, it is okay. In that case, you're looking for the Clomid to give you a "stronger" ovulation (ie, more mature egg that's more likely to be able to get be fertilized and implant).

    And GL!! PPs gave great advice. I hope it all goes well!

  • I took both Clomd and Femara. Clomid gave me hot flashes and I never ovulated with it. Femara gave me headaches but I did O.

    I wasn't monitored on Clomid. I was on Femara, around Day 12, before triggering. No bloodwork and just that one ultrasound per cycle.

    Yes, I always ovulate in my own and took it with all my TI and IUI cycles. It gives you a better/stronger O and possibly and maybe a second egg/target for the sperm.

  • I did not respond at all to Clomid, but I did respond well to Femara. I didn't really have side effects with either besides some minor hot flashes with Clomid. Good luck!

  • i had crazy side effects with clomid but none with femara. my ob wouldn't switch me from clomid to femara. when i switched to an RE he immediately took me off of clomid because he felt it could be increasing my risk for loss since two of my three losses were clomid cycles. i always get a baseline u/s and then return on cd11 or cd13 to see how things are progressing. from there i either get instructions on when to trigger or return a few days later to follow up and see if i am where i should be, if i wasn't before. i don't get any bloodwork done.

  • I am so glad for this update girlie! I am glad you are moving forward. Everyone gave awesome advice so far. Good luck! 

    I can only weigh in on the pelvic u/s, but mine looked at the state of my ovaries. That was when they discovered cystic ovaries and lack of/weak ovulation. 
    Congrats to my TTC buddy Madeline! Cheering for all the strong 3T ladies!

  • what e eryone else said plus at the U/S they will also look at you AFC (Antral follicle count or an estimate on how many eggs you could produce). Good luck lady.
