Dental Bill

Is this a bill? Or a statement of what insurance paid? It was a preventative check up which should be covered but they did prescribe medication because I had an infection. I asked the billing/receptionist person if I owed anything and she said no. I don't mind paying but there is no due date and it says statement (instead of bill).

It's a bill. But it doesn't look like they've submitted a claim to your insurance (or did so improperly). I'd call the billing phone number and make sure they have your correct insurance information and ask them to submit the claim. Any time you have something submitted to your insurance, you should receive a statement directly from your insurance provider about what they covered and what you should expect to be billed.

I think its a bill as well. You should have received an EOB at some point from your insurance that summarizes what they paid out and what you are left with.

It's a bill. Call them and ask if it was billed to insurance. It doesn't look like it was because normally it was say that insurance paid zero. It's probably just a quick fix.

It's definitely a bill but not the original bill, since it just indicates a balance forward and no services. I would call the dentist and see if a) they submitted to insurance and b) ask for a copy of the original bill that shows what the $58 is for. If you have any kind of FSA/HRA/HSA that you'll be using to pay that $58, you'll need a full bill if you're ever audited, not one that just shows a balance foward. The dentist should also be able to send you a copy of the EOB (since both you and the provider should get a copy) but if they won't, then contact your insurance company to get the EOB and make sure everything matches, once you get the bill showing the services.
