How far is office/hospital from your home? 10.67 miles according to Yahoo
How many hospitals are in your city (& what "names" are they)? I go to Aurora, but we've also got Prevea, Bellin, & St. Marys (which is now partnered with Bellin I think).

How far is office/hospital from your home? Within 5-10 miles
How many hospitals are in your city (& what "names" are they)? We don't have any hospitals in our town, but they are cuilding one right now and the closest one is less than 10 miles away
Do you have health insurance? Yes.
Through what company and how (MH carries the insurace.). Oxford (it is a division of United Healthcare and I LOVE it. They have been amazing.).
How much is your copay for Dr's visits? $10 for PCP/OB/GYN, $10 for specialists.
Is your primary Dr (obgyn/PCP) at an office or hospital? Both my primary and OB/GYN are in offices but are affiliated with the same hospital.
How far is office/hospital from your home? 20 minutes
How many hospitals are in your city (& what "names" are they)? There are so many in our are: Morristown Memorial, St. Clare's, Overlook, St. Barnabas, Newark Beth Israel. Those are just the ones within a 30 minute radius (20 miles or less). About 35 mins away is Hackensack Medical Center which is excellent. And NYC is literally right over the river with fabulous hospitals. All under an hour away (as long as it isn't rush hour).

How far is office/hospital from your home? 12 miles maybe
How many hospitals are in your city (& what "names" are they)? none in my town, but there are several in the state and well it's a small state so none are more then a 20 min drive

How much is your copay for Dr's visits? $20 normally but $40 for specialists and $100 for ER visits and none for my GYN

How far is office/hospital from your home? about 5 minutes
How many hospitals are in your city (& what "names" are they)? well Montgomery Regional is here and Carilion is just as close even though its not in the same town because we can hop on the interstate right by our house and its only a couple miles south...it takes about 10 minutes to get there

How many hospitals are in your city (& what "names" are they)? There is just one in my city I think (Rush Copley) but there are a few within 10 miles or so
