Marriage license help

My dad just called me in a tizzy.  His health insurance wants a copy of my parents' marriage license and he can't find it anywhere.  He even looked in their safe deposit box.  I'm guessing that they (ie my mom) lost it right after their wedding-39 years ago!  So anyway, I told him that I'll order a few copies for him (he'd call me every few minutes asking for help if he tried to order them).  But Hamilton Probate Court has two options:

Application for Exemplified/Authenticated Abstract of Marriage

Speaking of insurance, did anyone else need to send in a copy of their license?  I put dh on my vision & dental insurance and I signed him up for his own life insurance through my employer.  I was never asked for any proof of our wedding and we don't have the same last name.  He could have been on my insurance for years!  (kidding-that would be insurance fraud.  I'm not going to jail for that!)

  • I am totally guessing here but most places that have requested a copy of my license requested a "certified" copy so based on that verbiage I would vote for option one.  Can you call the probate court and get an explanation of the difference if the website does not offer one?

  • I went through an insurance audit a few months ago.  I'm not sure on the license thing, but I think a call to the marriage license bureau would answer that one.  The agency that did mine gave instructions on where to go for the various pieces of info they asked for (license, 2009 tax return, and B's birth cert).  I mailed in copies of those things (financial info blacked out) and received a postcard a few weeks later saying that I'd met the requirements of the audit and to save the postcard. 

    I didn't need to do anything other than sign a form for DH to be added to my health & dental ins, and when I added B, I needed his SSN# and date of birth.

    This sort of thing is a PITA but I kind of get it- insurance is expensive and even finding 2 or 3 people who aren't supposed to be on the policy can save tens of thousands of dollars.

  • I had to do this last year and I just went to Warren County's office and asked for it. It was $2.00 and took about 5 minutes to get!
