Jen S and Alicia: kind of legal question...
I won't hold you to this advice I am really looking for your professional opinions since you are a mommy and mommy-to-be. :-)
My parents are watching Dex while Paul and I are in Ireland. Do you think that a medical consent form is all that I should prepare for them in the event that they need to take Dex to the hospital/doctor?

My doctors office has a consent form that I filled out prior to going to Aruba that gives anyone that brings Chase in authorization for care. I just had to send in a note to school to notify them of our days away and gave my authorization for my Aunt who is the school nurse to handle all medical issues. She also had a copy of my insurence card. If its a simple doctors visit for a cold or ear infection you will need an authorization form. If its an emergency room visit, they will never deny a small child medical care. If anything they will send you a bill later and you can turn around and submit it to your insurance company. Which we have had to do before as well.
Chase needed a tetnus shot one weekend we were in Ocean City and hour and a half from home. His dad took him without a problem since we had been there before and they had my insurence information on file. They did bill me, just in case the insurence wasnt current and I submitted it to my company and that was the end of it.
that form looks good. i know i did one for a client before, i'll try to remember who and send it to you. it is more of a medical power of attorney. i would definitely have it notarized and even have it witnessed if possible.
