I'm so glad that you feel "right" about the next steps. Obviously it sucks that you have to make those choices, but I'm glad that you have more answers, a plan, and a hope for the future.

This. From what you wrote, it seems like Dr. D now understands that you're a seriously informed patient now and should treat you as such. He knows you understand your options and what the risks might be so you can proceed with eyes wide open.
