My coworker has f'ed up so bad I'm so mad I could spit nails. She had a huge order with a rush and she forgot to send through the proper form to increase the qty. So now the customer has it and she can't send out her invites because she didn't get enough. She's under my supervision so not only does she look bad, I look bad too. I'm so glad I'm leaving, I can't babysit these morons any more. There's no way I could have anticipated this happening but I feel like big boss is going to think I trained her badly. They're going to feel really good about giving me a recommendation now.
We're slow this month and she's alienating what could have been a long-term customer. Great job. I seriously had to walk out of the room when she told me so I didn't strangle her.

I won't get blamed because it was after the initial order was placed and the request was communicated via e-mail so there's no way I could have known it even existed, but I don't want them to think I'm a crap manager and trained her badly. I try to teach them to vigilant and careful with details, but I can't babysit them constantly. And do I seriously need to tell you "Don't screw up the $3,000 order?"
That totally sucks. And I fully understand as both my full time and part time jobs are all in the printing business.
When something like that, soemthing totally custom is messed up it sucks because someone has to eat that cost.
