I think I've been making a major faux pas regarding my references. I've been listing people that I have asked in the past to be references, but I haven't been contacting them recently to get their approval. My references are all a year old or older. So should I be emailing them now to ask again for them to be my references?

Definitely. Even though my references know I am actively job hunting at the moment, I still give them a heads-up when I get far enough in the interview process that they call references to let them know XX person from YY organization may be contacting them.

I haven't gotten to that point in the process where they might be contacting my references, but are you suggesting that in addition to asking them to be a reference again, I should also give them a heads up every time I think they might actually be contacted?
Yes and yes. They are doing you a huge favor by being a reference, give them the courtesy of a heads up.
Plus your heads up makes sure they know a call or e-mail is coming, so they can prepare what to say about you in advance and not be caught off guard, and so they know to answer a random number calling their phones/e-mailing them.
