Legal Forms Help - HIPAA Authorization & Waiver
I am working on starting and creating forms for my hubby and I, I have had great luck with locating free samples of all forms (DPOA, HPOA and Living Will) except this new HIPAA Authorization.
Does anyone currently have one that they are using that they would be willing to share with me ? --- of course blank without any personal information.
