Before I started working at home, my boss quoted me a salary and stated he would increase it by a stated small interval per year. I just remembered this and he didn't do it automatically (my pay is transferred from his bank account to mine each month). I'm sure it's just an oversight, although it makes me a little self conscious. I need to remind him to give me a 1099 for taxes and thought I would include a sentence reminding him about this fact. What's the briefest, least dramatic way of saying it? He's a surgeon, so he's not into flowery language.

Also, if you will be increasing my salary annually as we discussed, please don't forget to notify your bank of the change.

Re: Work advice
I like the way you worded it.
I'd also add something along the lines of getting a schedule in writing so you both have something to refer to in the future.
Ditto.
Also I'd maybe take out the "if" part. It sounds like there wasn't an "if" about it when you both decided on this and there's no point to add doubt, if that makes sense.
I like that. No ambiguity, but very pleasant.
