DH and I would really like to get out and do more camping this year. We have a tent (no RV) and are pretty basic campers.
Must Haves: Fishing access (willing to drive a little bit from campsite to fishing site), flushable toilets (my must have!), showers (esp if we are there for a long weekend), swimming (either lake/beach or pool).
We haven't gone to the boundary waters, so I think we should start there...but other than that, we're looking for new places to try this Summer.
We've been to Baylor Campground, Lake Auburn Campground & I've been to countless others growing up.

Just a heads up that there are no showers or flushable toilets in the Boundary Waters. The best you will have is a latrine- which is basically a toilet shaped stand that's placed over a deep hole in the middle of the woods. These are usually 150ft or so from your campsite.
We really like St. Croix State Park and Glendalough State Park and basically any state park along the MN North Shore. We're a fan of MN State Parks :-)
We really like Cascade River but I don't know how easy fishing would be. We're heading to Itasca State Park late spring/early summer this year and there are OODLES of lakes for fishing up there. This will be my first time at that campground though so I don't have any reviews yet.
We always do state parks too.
I can confirm that Lauree is right about the Boundary Waters. No flush toilets. No showers. There will be a latrine, and bring your own TP.
We've done St. Croix State Park a few times - tons of great hiking trails. Boat/swimming access on the St. Croix. My only complaint there is that the campgrounds are huge and can get a little noisy.
My favorite state park is Tettagouche on the North Shore. They have some awesome walk-in campsites that give you some privacy. The camp site is about 100 feet from your parking spot, so you have to carry your gear in, but not very far at all. I don't think there's a swimming beach, but there's definitely lake access for fishing.
Itasca State Park has all the things you're looking for.
If you want to stay a little closer than Itasca, Afton State Park is close and has everything you're looking for.
We're going to Glacial Lakes State Park for the first time this summer. We try to go to one new state park each summer. This one seems to fit the bill for you too.

Ooh! Let me know how this one is! This was one of the choices on our list last year and we did Glendalough instead.
We go to Interstate by Taylor's Falls all the time. We usually stay on the Wisconsin side, since alcohol is technically not allowed in MN state parks.
The Minnesota side has lots of sites right on the river that you could easily pop out and fish on.

We know the area ... my parents are from the small town that's closest to the park ... so I'm not sure why we haven't gone there before. My grandma lives really close, so we might bring her out to the campsite to make s'mores one night. :-)

We prefer state park campgrounds, too, and I'm a huge fan of the Lake Superior area. This book has been great for finding new campgrounds in that area and it tells you what type of bathroom facilities are available.
You might like St. Croix State Park. I'm not sure about fishing but it seemed like the swimming beach was nice when we were there.
Bear Head Lake state park is in Ely so you can be near the boundary waters but still have bathrooms.
