I think most guests will appreciate not playing shower games. If your sister hates them I say cut them.

For my SIL's baby shower, I found cute little plastic baby booties and baby bottles and filled them with m&ms and seasonal candies (it was around Halloween). They made for a cute decor at the shower and then doubled as a good favor.

We did cookies for my bridal shower and they turned out great. I also like the chocolate covered pretzels.
I say cut the games especially if that is what mom-to-be wishes. My mom and MIL didn't respect my wishes at my bridal shower and I was super p*ssed. Lets be honest, who wants to stick their nose in a diaper full of melted chocolate to guess what it is? ummm...gross.
If you have centerpieces or anything like that you can always do a raffle for prizes.

I say cut the games!!! I hate them and we will have none. The closest thing we are doing to a game is handing out pink and blue ribbons for people to pick and then having a reveal at the end.
At our wedding and baby showers we had no games...I don't think anyone minded at all.
For our wedding shower, we did Bath and Body giveaways (soap, little candles, lotion bottles) and gave away the centerpieces, which were potted spring plants (we drew recipe cards for the winners). For the baby showers, we just did cookies no larger prizes or anything. I personally like edible treats.
OMG a baby Pinata!!!! That is crazy.
We are doing a small vase and rose(s) at each setting with candy that can all go home with the guest if they like. It sounds nutty (like too much) but my close friend had it set up like that at hers, it killed two birds with one stone & people actually liked taking the vases home, not to mention it looked really nice.
