I am a total slack
@ss, and have not changed my name as of yet since getting married (our 3 year anniversary is coming up in June, oopsie poopsies!). Since we will be welcoming lovely Sprinkles into the world in 9 weeks (!!!), I seem to have finally found the motivation to actually do this, and hope to at least get my Social Security card sorted out this week. I've printed out the SS-5 form, have an original of my marriage license and my driver's license to take with to prove I am who I claim, but for those of you who changed your names, did you have to produce any other documentation to make the change? TIA!

Re: Name Change
I think that was it. I did the SS office in Portland with my marriage license and brought absolutely everything I could think of with me, but I think the marriage license was the only thing they needed. Then I hit the BMV to change my license.
Now, I have to do my passport still and think I have the motivation to do it. So I guess I need to go wrap my brain around how to do that.
I haven't done my passport yet either but I think it expires soon so that's a good enough time to do it-- I am cheap.
Ahhhhh yes- I am going to be that girl too! I can't bear to totally give up my last name (Irish clan pride and whatnot), but I also refuse to give up my middle name since I share it with my mom, so I'll be one of those PITA 4 name people. Although I plan on really only using my married LN in general and keeping my old last name on legal documents. Silly, I know!
Thanks all, now I just have to hope that I can get into the SS office right when it opens up and it doesn't get difficult!

I am a 4 name girl too... I also share my middle name with my Mom (and its my nana's first name... and we share a b-day) and I love my maiden name... I almost didn't change it so I definitely wasn't ready to get rid of it when I did decide to change..
I haven't done mine yet either. I'm planning to but just haven't found the motivation to go do it. I'm glad I'm not the only one. Also, I'm surprised that so many others are in my situation with the 4 names. When I do get around to changing it, I'll have 4 names, for the exact same reason as above.
