I'm applying for a new retail sales job and the application form asks for 3 professional references. I want to list a reference for the most prestigous retail job I had at a major jewelry store in December 2009. We haven't kept in touch since my temporary position there expired in 12/09. Should I email/phone her to request if she'll be willing to be a reference for me? Having her on my reference list is important to me since the position I had there was the most distinguished of my career, but I don't know if she'll say yes or not since we were on friendly terms but not close. Also I left the company in good standing. What's the best thing to do?

Of course you should. What would your other option(s) be? Use her without her knowledge?
ETA: Why would she delete it? Just e-mail her, re-introduce yourself, and tell her you'd like to talk to her about being a reference for you.

There are a million reasons that could have happened. It's worth a shot instead of fretting over the possibility of her not returning an e-mail.

I agree! It's worth a shot, she was your manager, after all, and it would be logical to use her as a reference.
This is also a good reason to keep in touch (at least marginally!) with your old contacts, especially ones that you might want as a reference!
