so one of the jobs i'm applying for requests a salary history - do i just add that to each position on my resume?

as just a paragraph? an extra page at the end? i've got 12 years of salary history . . . or something like "my requirements are negotiable and my most recent salary is __________"? (which is what i usually do when salary requirements are requested)
I've always been in the camp that you shouldn't give it up until they actually press you for it. The job I just took asked for salary requirements and history in the application. I didn't list the history, but rather included in the email I sent (not the cover letter, but the email to HR) "My salary requirements begin at $XX,XXX and are negotiable depending on the full package offered." They later asked me specifically the salary I left my last company with.
In general, though, I think it's the business of employers to know what you will accept, not what you were paid five jobs ago. So I'd be polite, but vague.
