Here's my situation, I have a passport card issued in my maiden name. I want to get a passport book issued in my married name. I can't find anything that says exactly what I need to do. I found that I can use form DS-82 to apply for a book since I have the card but what about the name change? Any ideas?

I need to do that too. I just haven't made it a priority since we're not going anywhere. I'm super jealous of your cruise! We are planning to go on one in 2013. Always planning ahead! haha

From what I found, the form DS-82 can be used to both change your name and apply for a new book. You have to provide you marriage certificate to prove your name change.

It depends on how long ago your current passport was issued:

http://www.travel.state.gov/passport/faq/faq_1741.html

a:You will need to complete Form DS-5504: Application for a U.S. Passport: Name Change, Data Correction, and Limited Passport Book Replacement, within one year of the issuance date of your current valid passport and submit along with the following:

The same data visually displayed on the data page of the passport;
The passport to be replaced
Certified documentation of your name change (e.g., marriage certificate, divorce decree with your new name); and
One recent passport photo.

After one year of the issuance date you must submit Form DS-82: Application for Passport by Mail, your current passport, certified documentation of your name change, one recent passport photo, and pay all applicable fees.

This is what I finally found. It was issued right before my wedding in 2009.

LB, you haven't had any issues with your passport not matching your other forms of ID? Our cruise is booked under my married name so I have to update now but if I had known that I would have just used my passport card for this cruise.

This is what I do. Yes Although I almost had an issue getting out of Ireland, so I may change my tune before our next trip.

I've never used any form of ID except my passport. Passport and cruise docs match so I've always been good. Even if not, my maiden is now my middle name so it's still on my DL. I don't know how legit that is, but I haven't had a problem the last three years.

Gotcha. I kept my middle name so my maiden name isn't on anything else.
