I'm filling out my preadmission paperwork and there's a yellow form that says "Maternity Reservation (Not Elective C-Section)"

There are no instructions that tell me what this form is for and if I need to fill it out (I am not having an elective c-section). The main preadmission registration form already asks about my due date so I'm not sure why I have to fill out a similar form.

So do I need to fill it out? There is a spot at the top that says "Reservation Date". Should I fill out my EDD in that spot? And what does EDC stand for?

Yes, I did call them and left a message but I kind of want to get all this paperwork done so I can mail it today.

I know exactly what you are talking about. I didn't fill it out and I didn't have any problems. They had all of my information. Don't send it in.

oh and also, don't send in the anestesia form. They will have one there for you the day of if you decide to go the epi route.

Thanks HW. I actually have to have a consult with the anesthiologist before my delivery so I planned on taking the paperwork to them because I'm not clear on all of it.

I did not get any of these forms. Did you get these when you went for a tour of the hospital or something? We have to still set that up...

No, my OB gave them to me at my first prenatal appointment.
