That's pretty accurate. I can hold on grudges pretty easily sometimes, and I feel apologies aren't genuine if they are only asking for forgiveness or regretting that they got caught.

Sounds about right to me! Ideal apologies, for me, should contain all of the first three in near-equal doses and a splash of the fourth.
The fifth, like one of the earlier replies mentioned that I'm too lazy to find out who said it right now, just comes across as SO insincere.

This took a lot longer than the other one. Maybe i'm just tired.
Mine were:
Making restitution (i'm not surprised, since i really think the best approach is to learn from one's mistakes)
Expressing regret
Accepting responsibility is important to me, but it's best paired with the two above; genuine repentance is similarly ranked.
I feel like asking forgiveness is just asking for a favor after messing up. Big fat 0.
