At a restaurant, if it comes first, I'll eat it first.
If I'm making dinner or eating somewhere that I can pick, then I will eat it last. When I was in middle school we participated in a cultural immersion project and spent time with "authentic" Italians who told us it was custom to eat salad last as a palate cleanser and to keep things moving. True or not it stuck with me and that's what I've done since.

Same here. This is SUCH a bone of contention between me and rrr. I grew up in an eat salad with dinner family. He did not. I think his mother started this because she did not want to deal with three boys/guys whining that they were hungry and stuck salad in front of them to appease them until dinner was ready. I still eat mine while I eat my dinner, whereas rrr lets his dinner get cold first.
On our first visit back to FIL's house after MIL died we had family over for bbq ribs. FIL made the ribs, but I got put in charge of coordinating (cooking) everything else. I had everything ready to go and on the table, hot baked beans, etc., and FIL wanted to eat salad first. Back the warm stuff all goes in the kitchen because I wasn't going to let it get cold on the table. UGH!

This depends on if I'm out or at home and what type of salad primarily dressing is on the salad.
If I'm out I'll eat it first since in most cases it comes first. At home it depends. If I have a salad with Italian dressing or Vidalia onion I will usually eat that with the main dish as I think the flavors of the dressing usually enhances the entree. Something like a salad with ranch or blue cheese could get eaten before or after. I'm not picky about when. Depends on which one I want more at the time salad or main dish.
