I usually try to be pretty understanding and if it's something that I'm just being worked in for (really sick, need meds now!) I'll wait however long it takes.
But in the case you described I would have walked out then and there.
I will wait forever at my ob/gyn's office because they keep everyone informed of emergencies and stuff - like Dr. Gogola is running X minutes behind due to an emergency delivery or whatever. Fine - I get that.
Had the lady said - oh, we had an emergency appointment or even I am so sorry, Dr. ___ is running behind today, it shouldn't be any more than ___ , then I would have been more inclined to stay. But nope...so I asked for my referral card back and walked out.
Pretty much this. Also, I'll wait longer if it's something like an OB/Gyn or pedi appt where the Dr. may in fact be delivering a baby or doing hospital rounds or something. Dentist, no way, they could at least get you into a chair and get your teeth cleaned and xrays done while the dentist finishes up whatever he's doing.
Based on your scenario, 30-1 hour. But with that response, I would have been pissed and would find another dentist.
Once I waited at the OB for much, much longer. Like 2 hours. But she had and emergency c/s. After the first hour I decided to stay because it would be just as much of a PITA to make another appt the week after and miss work again. It was the end of the day, so it didn't really matter. If I need to get back to work, its a whole other ball game. If I'm with C, 30 min would probably be too long! His meter runs out after about 7 minutes. :-)
I left the pedi office a few weeks ago after waiting one hour with a cranky kid. They tried to reschedule and apologized but it made no sense when people who came in after us were going right in.
I am beyond over doctors double/triple booking. It makes me want to pay a concierge fee for better service. They get one more chance in my book and then we'll switch. I have no loyalties.
In your scenario, I'd have informed them I was leaving and would not be back.
When I'm with my mom at MDA, there is absolutely no telling how long we'll be waiting to see a doctor... but when her doc finally comes in, we know we will get the time we need with her. I appreciate this and would wait all day. We are generally waiting on her labs and imaging to arrive to the nurse's station so the doc can reference them.
My specialist will respond to me in an emergency and keep other people waiting, so I appreciate that sometimes I'll be kept waiting. Luckily, their office is awesome and calls to let me know if he's running more than about 45 minutes behind.
My general practitioner or GYN would see me reschedule if I had to wait more than an hour. I try and book the earliest appointment o the day, or the first one after lunch when I book with them. It never ceases to amaze me the number of children some people will bring with them to the OB appointments! I don't like to be in that waiting room for very long because it tends to be chaotic.
Once they get my weight and BP and stuff, there's a separate waiting room for patients only at the GYN office. I'd wait in there all day. It's the quietest, most peaceful place ever, and full of magazines I'd never admit to reading...lol.

I would also wait at MDA. But for an endodontist for a root canal that needs to be re-treated or yanked like I was waiting for today - I was furious. Mostly at the "do what you gotta do" response I got when I said I couldn't wait much longer because I had to go back to work. Grrr...
