driving (particularly on the highway) behind a truck/van with ladders strapped to the roof. I often envision the ladder coming loose, crashing through my windshield and either decapitating me or penetrating through my chest.

And I don't like driving behind or next to semi trucks.
A girlfriend of mine was on her Harley when a ladder came off of a truck. Luckily, she was a nice, safe distance away and was able to miss it. Scary!
I have the same thought/fear. Though it's pretty much with anything tied to a roof.
My ex in highschool was following a van that had a mattress and box spring on it's roof, apparently it wasn't secured, it came flying off, through his windshield, and gave him a concussion and two black eyes, and a giant gash on his passenger's head.

I was behind a truck once, who had a refrigerator laying down in the back of it, totally unsecured... they stopped abruptly and I'm glad I was a good way behind them because the refrigerator shot out of the bed and stood upright in the middle of the road! Poor guy was all alone trying to get it back in the truck, haha.
