Quite a few of the Eastern and Southern Caribbean ports offer these. I know that there is a Rainforest tour in Dominica and I believe there were waterfalls there. There is Dunns River Falls in Jamaica that is a big waterfall over rocks that you can climb. Ziplining is available in many ports as well.
You can look on the cruise line websites for the different itineraries and then look up shore excursions for those itineraries for an idea of what those ports have. You can, of course, book them on your own if you prefer but this just is an easy way to see what is available.
On our recent western caribbean cruise, we went ziplining in Honduras and I highly recommend it!
On our southern caribbean cruise a few years ago, we went on a waterfall and river tubing through the rainforest excursion in Dominca, I highly recommend this tour as well.
