90% of the time, I assume they don't want to be bothered and will just email or IM them instead. Unless it is extremely urgent - then I will knock and wait; occassionally I will knock and enter if they ignore me and its super urgent (this has happened like 3x in 6 years)
One of my employees regularly walks into the office when the door is closed - its usually b/c I'm eating, but I have had to yell at her about this a few times since my door doesn't lock and its conceiveable I could be changing or something. But, she has horrendous manners and clips her nails at her desk, so she's a bit of a lost cause anyway.

I always knock and wait for a response. I don't assume the door is shut for some major reason - it's usually just to cut back on noise since there can be a lot of foot traffic in some areas of my building.
