Following up: a three-part question
I interviewed for a dream job 2 weeks ago today. As I was leaving, the hiring manager told me they thought they'd have a decision made in mid-May. So, when is it appropriate to follow up, what should that question sound like, and should I start with the HR person or the hiring manager?

I would probably follow up around May 21st if you don't hear something before then. My idea of "mid-May" is around the 15th. I'd give it a few more days just because emergencies happen and people get sick.
I would contact the hiring manager if you have their number and follow up with them. A lot of times the HR won't have any contact with you until you fill out new hire paper work. When talking to the hiring manager I would just say something like "I am just checking in to see if a decision had been made in regards to xxx position." I wouldn't have any problem telling them you are still very interested in the position.
Honestly, I'd contact the hiring manager now. But more of a "I know you haven't made any decisions yet. I just wanted to touch base and see if there is anything else you need from me, any other questions you may have that may help you in your decision making process.".
Just to make sure you stay on their radar.
