grimms can see their true identities even when they aren't in that form...but if they're in their original form, i think anyone who sees them can see it.

I think this is true, but I also have a further question. Why is it that sometimes Nick can see who they are right away, but other times can't? Like in this week's episode, he knew who the godfather guy was right away, but not Lucinda. And he saw her, so you think her true identity would be revealed. Maybe he has to sort of scrutinize people?
This has been bugging me too.
I think he sees them for what they are once they're experiencing a strong emotion. I've noticed that a lot of the time, there's usually extreme anger or fear when he seems their other face. In Lucinda's case, she had Spencer teaching her control her WHOLE life so she was able to keep her emotions in check and not creature out in front of him. Her grief was fake so he couldn't see it

That's a good idea. That has been bugging me too and you answer does fit.
Does anyone know how the creatures know that Nick is a Grimm? Because that has been bugging me as well. At first I thought it was because he might react to their face change thing. But he doesn't always seem to have a reaction? Which makes me wonder if they know he's a Grimm because he somehow looks different to them?
Also, I think they addressed the whole other creatures and humans thing a few epiosdes back when Nick and Monroe talked about telling Julia. Nick suggested that Monroe change in front of Julia and Monroe said that people don't react well to that. It either really freaks them out or their brain sometimes refuses to believe what they really saw.
My guess is that once they have their other face on, there's probably something different about Nick that they notice (smell, reaction, maybe he has a glow about him or something).
I think it's his reaction though. There was an episode where he hid his reaction pretty well and the creature didn't realize he was a Grimm right away (I think).
