i don't know if anyone can help but if if you can that would be great! we are planning a to go to jamacia for our friends wedding in october 2009. my passport expires september 09. i haven't yet changed my name on my passport. do i have to apply for a new passport? what do i need to do. please help me.

Yes, you have to apply for a new passport since it expires and you have to change your name on it.
The only way that you didn't have to pay for a new passport was if your passport was less than a year old when you wanted to change your name. Since it's expiring in 09, that wasn't the case any way so no matter when you changed your name you would have to pay.
I did everything by mail and had my new passport back in 2 weeks without rush. All of the paperwork is online at this site. You also need a marriage certficate and your old passport - you get both back but under separate envelopes. You would do form DS-82.
