Pet insurance

Does anyone have pet insurance? What do the standard plans usually cover... vet visits, medicine, etc? Do you just have to pay a co-pay for visits? Would a UT infection count as a pre-existing condition? If you don't mind sharing, what plan do you have and how much does it cost you per month? Thanks!

I have emergency coverage for my pit bull, who tends to eat things that are not food.
We didn't find the wellness plans worth it - we visit the vet once a month for a checkup, and as needed for things like ear infections. We go to a low-cost clinic for heart worm tests and vaccinations. We'd pay more in insurance premiums than we would just paying out of pocket at the vet.
