RSVP how?

So I got an invite to a friend's wedding for my boyfriend and I, we of course are going!
But I'm unsure how to fill out the RSVP seeing how this is the first wedding I've been to that I wasn't in.
The invitation has:
M_____________________
____ accepts with pleasure
____ declines with regrets
number of guests attending ______
I guess my question is should I put mine and his name on the line or just one
and if I put both names should I put 2 guests or If I just put my name should I put 2 guests?
I know this I sound dumb but I'm super confused.

I woldn't ask, either, if I could bring a guest.
Your name goes on the line and you check off "accepts" Zero in the place for the guest.
