Help! Change of Last Name Question!
Ok, so here's my back story. I've been married for 2 years now and I've
been meaning to change my last name for FOREVER. After my husband and I
got married the pastor who married us kept filling out the paperwork for
our marriage certificate wrong. When he FINALLY got everything right
he'd already filed the paperwork THREE times. So by the time everything
was finally processed and we actually received our certificate it had
been months and we were almost in the new year. So life happened, other
things took priority and the change of last name took a back seat. So
here we are 2 years later and we're about to have our first baby. He's
due in the middle of November and I want my name to be changed by then
so that my married name can be on his birth certificate. My question is,
how many forms are there to complete the process and does it cost me
anything to have to change my last name?? I already filled out the SS-5
form for my new security card. But I saw on the mass.gov
website that I need to fill out a Change of Name form for Probate and
Family Court and there's a fee to do that??? Is that necessary? I always
thought that all I had to do was my Social Security first and then
DMV?? I would really appreciate an answer! This has been stressing me
out and my husband and I have been so busy trying to get ready for baby
that it wasn't until recently when my doctor handed me forms for the
hospital and birth that we remembered about my name change!
