Hi ladies. Just a quick question. In order to change my name (I'm a newlywed) what do I need to do? I just received the certified copy of everything that I need. Are there forms that I need to fill out? Or can I just go to the SOS office? Any other places I need to change my name that I'm not thinking of? TYIA!!
Re: Name change
For the SS office, I took my old SS card, birth certificate, drivers licenses, & marriage certificate. I think they looked at everything.
For the DMV, I waited till I had the new SS card, I went to the one on Foster & Elston on a Friday around 11am, I was in & out in 20 minutes. Took all of the same stuff as above.
There is a name change checklist on the NEST!!! Just go to the homepage and there is a link to get a bunch of different checklists from changing your name to buying a home.
I changed my name,
I had certified marriage license, current id and birth certificate with me.
I first went to the dmv, went to the express center downtown. then proceeded to the social security office where you should already have the form ss-5 filled out, or you'll have to it there. Make sure you go early if you go to the one downtown!
