The docs adjusted the amount of Ritalin he is taking, and I have to admit, the child is like an all together different little boy. He is so good now, and he listens and I hate to admit it, but I may have been wrong about the use of the drug.
I am not a psychiatrist, but even I can see that he is so good now and so much happier. He is cuddly and watches movies (without destroying the room).
I hope it all works out for him.

I had the same "now I believe" experience with ritalin and my brother when we were growing up. The thing that really sealed it as the right choice for him was the fact that HE was happier, and could more easily do the things HE wanted to.
I agree that Ritalin and other stimulants are overprescribed in this country, but that doesn't mean it's not the absolute right course of treatment for some people.
Here's the interesting thing- I think that him being calmer has allowed my brother and SIL to change the way they parent him. before when they said no, he looked at them like they were insane. He was completely defiant, but I dont' think he wanted to be.
Now, they say No and he stops what he is doing and listens. He is more verbal with his wants (and the kid has one hell of a vocabulary for a 3 year old).
The way I understand the mechanism, kids who respond in the intended way to Ritalin are ALWAYS insanely bored without it. Their brains get less excited about everything they do, so they are always, always trying to find something to ease their need for something interesting. The Ritalin is essentially speed, so it makes every little neuron fire just a little bit more each time you do something. The boredom feeling disappears, and they are more content to do one thing at a time. I'm sure your nephew's parents telling him "no" felt to him like they told him he had to stand in a corner and hold perfectly still for no reason.
It's actually the same thing coffee does for the rest of us in the morning - the incredibly important meeting you're in seems more boring because you're tired, you start to fall asleep. With a cup of coffee to help, the caffeine increases the amount of stimulation from everything you percieve, and you can stay awake through the meeting without being miserable.
