Lately, four or five months before. The big trips maybe 6+.

As soon as I can make the plane reservations. We use my frequent flier miles and those tickets go fast. So as soon as we've decided on a destination and time of year, I put it on my calendar.
For days off here and there, as soon as I know.
depends. i actually have a trip planned for April, because that's when we'll have time and money to go. but i'll probably end up cancelling if anything crazy comes up.
when i worked in leisure travel, we'd go whenever i got a super deal. usually, within 3 weeks.
That is my nephew Luke. I absolutely adore that kid, and I hope when I have one, he/she it anywhere near as well behaved and happy as he is.
H and I are in talks about what our next vacation will be - it might be anywhere between June and September (if we hold off for our 1st anniversary). I keep trying to talk to him to get an idea now of what we want to do, so I can start researching/budgeting for it, but H doesn't think there is any rush. I'm definitely not ready to figure out any real details, just a when/where/how long.
dh and i plan at least six months out for vacations a week or longer, including flights, hotel and car rental. by the time we actually travel, everything is already paid for.
shorter vacations or weekends away are planned anywhere from a month to four months out, depending on location.
we save for vacations year round.
We had to pick our vacations in January. So, every January we'd decide on a week vs a long weekend then decide how it fit into our on-call schedule. We may not pick a final destination until a month or so prior though.
We also save year round.
4-6 months out, unless it's something like DragonCon or other conventions which are announced a year in advance with time-sensitive price tiers.

Heh, on the list of our potential vacations are San Diego or NY ComicCon, so I'll pull up a Geek chair with you.
