Do I ask Boss Lady about it? I really don't think she realized it. I don't want to hurt her feelings, but at the same time, I know she'd think it was hilarious, too. Hmm...
Re: Here's the other thing.
If it were me, I would admire the bag in front of her, then pull the changing pad out and say, "Hmmm...what's this?" I would lead her to realize it on her own, but in a way that shows that you still enjoy and appreciate the bag.
You can also mention that the waterproof pouch will come in handy if you ever pee yourself.
