So in a couple of weeks we close for our house. So finally i take the chance to change my lisence like that i wouldnt have to pay double fee in terms of when i change my name pay a fee and then when i would move pay another fee so i decided to wait and do it all together when i finally have a house so now that i'm about to get one i want to make sure that i do the process of name change right.
I went once and they told me that i had to change my name first in my passport and then i was able to change my lisence is that true?
What did you guys have to bring to change ur name for brides that married this year??
Any good offices to go to i live in miami and i'll be moving now by FIU
Thanks girls!
Re: Change name @ DMW
I went to the SS office first, you have to fill out form SS 5 which you can find here: http://www.ssa.gov/online/ss-5.pdf
I went to West Palm Beach office and I was in and out in about 15 minutes. I went on a Tuesday around 11AM.
Then you go to the DMV- I would highly suggest making an appointment if possible, I was in line for 3 hours there because I didn't have an appointment.
You need: a certified copy of your marriage licence, your SS card, your birth certificate, and two pieces of mail with your name and address on it (I think I brought a water bill and a renewal notice for my insurance.)
Here's the link to make an appointment at the DMV: http://www.dmvflorida.org/fl-dmv-appointments.shtml.
Here is link for the correct proff you'll need at the DMV: http://www.flhsmv.gov/ddl/faqkeys.html
Also, I was able to change my name on my credit cards online by sending an email to customer service, and they didn't need a copy of the marriage certificate.
Your bank will need you to come in person and bring your marriage certificate and SS card to change your name.
Also, make sure when you request your name to be changed on credit/debit cards you ask them to send you a replacement with your new name. Also make sure to order new checks.
I did all of this in April of this year and I'm glad it is over! GL and HTH
