name change help!

OK, so I think I am ready to finally change my last name.

 

But, I am not 100% sure on how to do this.

 From what I understand I can dl a name change application from the social security office and mail it in?

 Then change my dl and banking info?

 Any help appreciated!

  • You can get the application online but I dont think you can mail it in for security but not 100% positive when I changed mine I went down to the office and they took my old social away and mailed me my new one. Then they notify the dmv and you have to go 24hours after to get your new license which takes forever. You have to wait in the regular line for an id change and then they take a new pic and you get your id right away. its pretty easy and not too time consuming.

     You are allowed to mail in the application, but do I really need to put my parents ssn down?  Or can I mark unknown? 

  • I downloaded that paper and filled it out and everything, now did the lady ask for it? No, I know this might not be the case with everyone but that is what happen to me. I even filled out the parents part too.

    Its really easy, just fill it out, take your marriage certificate and your ssc. I think the worst part that I hear is the waiting in line at the SS Office.

  • You're changing your last name, right?  I'm pretty sure you can't mail it in, you have to go to the social security office with your current social security cards, driver's license and marriage license (has to be the certified copy).  I don't recall having to put my parent's ss# at all.  They take away your old ss card and mail you the new one.  A day or two later you can go to MVD to get your new driver's license.

  • yep if you live in/near phx you HAVE to go down to SSA, no getting around it.

    but I did fill out the form completely and handed it to her, it went much faster because of it she said

  • Definately remember the *certified* copy of your wedding certificate.  It's easy, it just takes a little time to get it all done.
  • Also as far as the banks go, I just went in and changed my name. Then I had to call all my CC companies and change them as well which was pretty easy. I hardest thing was changing my freaking email at work. Good Luck, it's not hard at all and when you get your new ID it's fun.

  • When I did this a year ago, I had to bring my old SS card, the forms completely filled out with parents SS# (I think they use it to verify you in the system) and the certified copy of my marriage license. They took my old card and gave me a print out to take to DMV. I was told to wait 2 business days before applying for the new license. I went to the SS office on a Tuesday or Thursday at 2 and I was out of there in less than 20 minutes.

  • I changed my name in April and it wasn't too bad. I also didn't know my dad's SSN and they didn't give me a hard time about it either.  I had to go twice to the bank to change my name though because if you have joint accounts then the other person also has to be present to "sign off" on the name change as well, weird I know and it was frustrating that the online instructions never mentioned it.  I also changed my name on my passport, that was what took the longest (I think 6 weeks or so), and I couldn't travel anywhere while waiting for the new one so keep that in mind.  but their forms I was able to fill out and mail in along with the marriage certificate, payment and my old passport.
