Won Nest Contest - Affidavit??

Hi Ladies, 

 I just received an e-mail saying that I won the nest's body cream contest.  I am so excited because I have never won a contest like this and think it is pretty awesome!!! However, I am a bit concerned that they have asked me to fill out an affidavit and get it NOTORIZED.  Thought that was a little weird.

 Have any of you done something like this before.  Do you think it is safe??

what does the affidavit say?

I did reply to the email to ask them about it.  They said all giveaways require an affidavit for legal reasons.  To protect them I guess.  It just says stuff about that I am who I say I am, holding them harmless and that they can use my name or likeness in advertising.  That last bit is the only one I am really worried about because I really don't want my full legal name broadcasted on the internet.  I am fine if they use my screen name.  Maybe I should just clarify that with them.

Definitely ask them to change the affidavit to clarify that they would only use your screen name! otherwise, it sounds normal to me.

We won a trip to Hawaii in 2009 and we were required to sign and notarize an affidavit as well.  I believe it is standard practice for any prizes/sweepstakes.  Congrats on the win!

WOW you won a trip to Hawaii!!  That is awesome.  I was pleased with my body cream win, haha.  Well, everything is sorted out.  I e-mailed asking about that and they promptly sent me a revised affidavit that said "screen name."  Hopefully the wait is not too long!
