Keep in mind the train they took to the Capitol made stops in other districts. Plus, 13 and 12 were definitely farther away than others. That's why there was less government intervention early in the book.
It also makes sense that the Capitol would treat its immediate border districts better than the others, which leads me to believe it was only directly touching 1 and 2.

The Capitol was in the Rockies, that was specifically stated.
I assumed that 13 was around the DC area since it specifically alluded to basically being a bunker for important people pre-Panem.
