Changing name with the DMV

Hi Ladies,

 

I'm in the processing of changing my last name. I already did so with the social security office.  Next is the DMV.  I noticed that I can't get the form online but can pick it up in the office.  Does anyone know if I can just walk in and get the form before my appointment or is it something they give me during the appointment? If I can do it before, then I will leave some extra time, but there is no point going early if they won't give it to me until the appointment.

When I changed my name with the DMV I didn't make an appointment, just walked in. They gave me the form when I checked in and it was pretty easy and quick to fill out. The whole thing took me less than an hour. Be prepared, you will need to take a new photo

They punched a hole in my old ID when I went to change my name. 

Yes, the form is there and it's really easy to fill out. Also, I had to bring my original (color) my marriage license and they asked if I changed it with Social Security, but I don't think they asked to see it. 

And, no, you don't need an appointment. But unless you have time (it could take more than an hour) you can make an appointment and get out in 15 minutes
