Changing your name on Passport- HELP!

I feel like an idiot-

I'm stumped. It's asking for the date and location of name change. 


Is this the wedding date and location or the date I changed it with the SS office here?

There are no instructions for this one!

  • Ok so I'm answering my own question- I called and they said

     The date and location of your wedding.

  • Good to know! DH and I are thinking of taking a 1st Anniversary cruise in October-I JUST thought about this last night...that I'm gonna have to get that changed....sucky thing is, it costs the same as a new passport! :(

    Even if you change your name you don't HAVE to change your passport if you don't want to.  Just be sure you always use the name on your passport when booking out-of-country travel and keep a copy of your old driver's license.    Also I don't believe you have to have a passport for most cruises, either. 

  • Also, if you apply for a new passport within a certain period of time after the name change (I think it's a year) the fee is waived. I procrastinated and ended up having to pay the fee again because I waited a year and a half.

  • I've had my new passport app ready to send for 3 months and haven't, opps.  Now we're planning on going to Mexico in two months, so I'm just going to travel under my maiden name and change it once I get back.  It does suck having to pay the fee if it's been longer than a year since you got your passport!!!
