Faculty reference checks

I am putting together a reference check questionnaire for a faculty position. I've done tons of them for staff positions, but obviously this is a different situation. Does anyone have some good questions to include when checking references for a position like this? Thanks!

Re: Faculty reference checks
We use the standard, "Tell me how you know the candidate" but we also ask about their strengths and challenges in the three major areas: teaching, research, and service one at a time. Finally, we conclude with the general "Is there anything else you'd like to add." So, a total of 5 questions.
