I'll post all the info when I get back home this weekend.  It actually isn't Puerto Rico that I looked at but Costa Rica.

Sorry it took me a little while to get back to you.  The company that I looked at using is Passport Medical

    www.passportmedical.com

A couple other ladies on the IF forum have used this company as well to do Costa Rica IVF.  The website has a bit of information and the person (Michael) that runs the program is very helpful with any questions.  When Phillip and I priced out the whole trip with everything we were looking at about 9K for a month work of IVF vacation.  They would start in CR and then end in Panama.  There are other options such as to do the whole thing in Panama or to do the monitoring and prep in the US and the actual IVF in Panama.  The dr that does the procedure doesn't speak a lot of english so they have to be comfortable knowing this and using a 3rd person as a translator.  We discussed having a family member come with us that speaks spanish...just a thought.  There are also options in Europe, Turkey, Australia, etc that they can look into as well.  The Australia exchange is not as good as their dollar is pretty strong and the flights can be pretty expensive but if you time it right you can get some good deals.  I wish you friend a lot of luck as IVF is a tough pill to swallow and the cost can be a big issue...I know.
