Grandma t&p again..

I hate even having to write this out because it makes me so sick and sad..to make a long story short, my grandma was released to a nursing home for a two week after care stay..the next day she was found on the ground, with a gash on her head. She had had a stroke. Turns out she had also had a stroke while in the hospital, but it wasn't found on her mri because it happened the day after. Somehow she was still released to the nursing home. She's now back in the hospital. It's a horrible situation and it was totally preventable. My mom flew from Cali straight to PA yesterday. She's trying to figure out how this whole thing happened. So please send some more t&p her way. And my mom's. She's heartsick over it.
