Potty issues in a 4 yr old

Anyone have issues or experience with a older child that is still having accidents?  I have a 4 yr old girl that continues to pee on herself.  We have tried everything from sticker charts to different reward systems.  I have consulted with her pediatrician and they sent us to a potty specialist urology nurse.  We tried her program with no success.  She had us use a watch that went off every 2 hrs. and rewards.

My daughter ignores the watch and isn't very interested in the reward system.  I make her clean up after herself each time she wets herself.  This doesn't phase her either.  She is a first born and very power driven so I am sure it is a power struggle.  I am about to back off and put her back in diapers or pullups for a little while.
