I'm applying for a job that has a web form as well as a place to upload a resume. I'm filling out the form and it asks for starting and ending salary at previous jobs, whether or not they can contact employers and why not if no. I'm new to job searching. I've really never done this. I've heard of potential employers asking for a salary range but never what your start/end is. And, I don't want to look shady but....I don't want them to contact my current employer. I've been here for 7 years, and while I've never burned a bridge with previous employers....I haven't stayed in touch, so I don't know if I want them to contact even older employers. It also asks for a "reason for leaving or wanting to leave." Are these normal things?

Those all sound standard to me.
I'd fill out the salary completely and accurately.
I think it's totally understandable to ask them not to contact your current employer while you're job searching and any employer would understand that. Same for previous jobs and losing contact. I'd click no to those and not think twice about it. If they have any reservations, they'll ask about it in the interview.
As for why you're leaving (which is totally normal on an application FME and be way prepared for more questions about this in any interview you have), why are you leaving? Then try googling tips for how to word your answer and how to answer that question gracefully in an interview. I'd link, but I don't want to google this question at work lol.
