Do you have to provide documentation for days off at work?

I had to take off two days last week, one personal day for a meeting at my stepson's school and another for his 2 doctor's appointments...I took off two weeks ago because I had a fever and I stayed in bed all day.  I'm nervous that they may say something, I have documentation for everything except the sick day...

What is your policy at work?

I've never been asked.  I think I might need a doctor's note after a certain # of consecutive call-out-sick days, but I've never been out sick more than 2 days so it never really applied to me.

No.  I had the flu last year and was out for three days - they didn't ask for anything.

If we're out sick for 3 days, maybe 5, not sure, we need a Dr's note clearing our return and stating std should not apply....if jury duty, then we have to provide a copy of the summons....otherwise, no.

We need documentation for being out for more then a certain amount of days. I had kidney stones and got the paperwork from my doctor but wasn't asked to turn it in.

Never, we only need a note saying we are safe to return if we are out more 3 days.
