We had a recent situation in which an employee on FMLA was unable to speak with us about the medical certification from her doctor - it was incomplete and we needed to address areas that we needed additional information in order to determine if she was FMLA-eligible. Her husband said she authorized him to speak with us - we did, not on any specific medically-related issues, just about the form in general and which areas the doctor needed to address.
We started thinking about having employees who are on FMLA designate a health care 'proxy' of sorts who can speak to us about the condition/form/etc. (similar to how the disability form allows you to designate someone to inquire about claims on your behalf) - do you happen to have something similar to this?

When I was in HR everything was done in writing by fedex, signature required. It doesn't matter who takes the form to be completed and returns it but this way you know it was received.
