I've only had one in my life (also as an adult), more than 10 years ago, so my memory is rusty as to exactly what went down for me that time.
Have you ever had (or heard of) an ear inf without having had a cold or sins inf, etc? Some sort of precursor? I got nailed with terrible ear pain out of the blue. I have been waking a little congested, but without meds and simply blowing my nose when I get up, I'm good to go....def no cold or sins symptoms.
Also, is it the norm to have pain externally too? If. Touch the cartilage around the ear opening (top, bottom or behind the ear opening), the pain is awful.
Lastly, have you had your ear drain? The pain was intense last night. I went into the bathroom to get some Motrin and my ear started draining fluid and a tiny bit of blood.
Im on antibiotics, but haven't been checked by my dr (he called in the script since I'm nursing, based on my symptoms....before the draining started, rather than have me bing my newborn to an office full of sick people).

This is a big indicator. I've had a few ear infections as an adult. They are awful. It does take a few days for the antibiotic to kick in.

but that said: Yes you've got an ear infection, the draining last night was your ear drum rupturing due to the pressure. Yes its going to hurt. a. lot. Yes it will hurt in the places you go mentioned. You can have an infection (esp middle or inner) if you have gunk in your sinuses. I know you said you weren't stuffed up but if you somehow got the stuff in your sinuses to back itself into your eustation tubes yes yes yes you can have an infection. I've been getting them all my life, the last one was about a year ago.
So:
1. CALL YOUR DOCTOR AND GET TO SEE THEM SOON!!
2. Take some advil to help with the pain.
