Temp Disability - did everyone get their 1099-G?
This is the only thing I need to finish our taxes.

Form 1099-G
The State of New Jersey is no longer mailing Form 1099-G, Certain Government Payments, to report
the amount of a State tax refund a taxpayer received. State income tax refunds may be taxable income
for Federal purposes for individuals who itemized their deductions on their Federal tax return in the
previous year. If you need this information to complete your Federal return, visit the Division?s Web site
(www.state.nj.us/treasury/taxation/) to view or print your 1099-G information or call our Customer Service Center at 609-292-640

I wasn't on disability in 2011, so they may have changed things, but I believe your employer is supposed to report your disability earnings on your W2. It is categorized as 3rd Party Sick Pay and should appear on a separate box on your W2.
Of course, if you were unemployed (or self) during your disability then none of this applies and I was not helpful at all!
